In a rare and complex medical achievement, the Liver Transplant and Hepato-Pancreato-Biliary Surgery team at PSG Hospitals successfully performed a living donor liver transplantation on 15-year-old Roshan M, who was battling Progressive Familial Intrahepatic Cholestasis Type-3 (PFIC-3) along with multiple life-threatening complications.
Diagnosed at the age of six, Roshan developed end-stage liver disease complicated by portal hypertension and hepatopulmonary syndrome, making transplantation the only definitive treatment.
After detailed multidisciplinary evaluation and high-risk consent, the transplant was performed with his 45-year-old mother, Ms. Priya, serving as the donor.
Intraoperatively, surgeons noted a cirrhotic liver with ascites.
The technically demanding surgery was completed successfully, and the patient was shifted to intensive care on ventilatory and inotropic support. During recovery, Roshan developed neurological complications including seizures suspected to be related to immunosuppressive therapy.
He was managed with medication adjustments and close monitoring.
On the tenth postoperative day, he developed severe sepsis with septic shock, which was treated aggressively with antibiotics, antifungal therapy and critical care support.
With gradual improvement in liver function and stabilisation of vital parameters, he was discharged in stable condition with good graft
function.
The transplant was led by Dr. B. Joseph John, DNB, MRCS (Edin), FRCS (London), Senior Consultant – Transplant and HPB Surgeon, and Dr. K.R. Kirubakaran, MS, MRCS, DNB (Surgical Gastroenterology), PDF (LTx), Senior Consultant and Unit Chief.
The team included Dr. Manoj Prabhakar, Dr. Niresh Kumar and Chief Nurse Rathi.
